The following forms must be received in our office prior to camp starting for your child. Your child(ren) will not be permitted to participate in camp if paperwork is not submitted.

The registration packet includes:

• Registration form (one for each child)
• Medical form
• Document Signature Sheet
  • Information to Parents regarding abuse of children
  • Discipline Guidelines
  • Information on Communicable Diseases
  • Policy on Release of Children
  • Expulsion policy
• Photo Release Form
